In general, the expression "birth certificate" can submit to both the unique document or to a official acknowledgment thereof. A birth certificate is essential evidence that credentials the birth of a child. Outside the United States, the term "birth certificate" refers to a certification of the original birth record. A birth registration is the conception of a permanent identity record.  It is a two-step procedure that commences with municipalities collecting the birth registration information from the parents and matching it to the information provided by the doctor or midwife. If the news collected from both sources is in agreement, the municipality forwards the documents to the Office of the Registrar General for registering. The birth registration form itself cannot be used as evidence of identity or to request documents such as Passports or Ontario Health Cards. On an occasion this form is recorded, a birth certificate or certified copy of the birth registration can be achieved and used for these purposes. Now the staff at glades general can assemble the patient data, print out the report, and get the required electronic signatures -- all at the bedside, within minutes.  It is also known as the electronic Birth Registration Information System (BRIS) was initiated  on a trial basis in Rajshahi City Corporation (RCC), one of the oldest municipalities in Bangladesh. BRIS is based on a distributed application architecture, with four clients and one server connected via a local area network.  It’s another name is Electronic Birth Registration System (EBRS). For the period of the process of preparing and finalizing a birth record, it is from time to time quite essential to save comments or remarks about the case. The comments can be easily collected by anyone viewing a case and can serve as reminders or as instructions to others who will work on the case. For example, if a definite case is a duplicate record, a remark should be introduced to designated to other users that the case is a duplicate and should not have any further data entered or be certified.
